Organizational Description
Community Health Systems, Inc (CHSI) is a Federally Qualified Health Center (FQHC) and Non-Profit 501 (C)(3), operating six community health centers in the tri-county area of San Bernardino, Riverside, and San Diego.

CHSI focuses on providing high quality and compassionate medical, dental, women’s health, behavioral health, and vision care services to those who are underserved, uninsured, or under-insured.

They offer services to meet basic health care needs, such as physical examinations and diagnostic screenings. If a patient has a unique problem that requires a specialist, their physicians will refer the patient to one and coordinate care with them. Family physicians treat patients of all ages, from infancy to old age. These physicians have the training to care for more than 90 percent of the problems for which patients consult doctors. Additional services include optometry and dentistry. CHS’s board certified Physicians, Physician Assistants and Nurse Practitioners provide comprehensive primary Care services.
